Take (or mail) your completed application and original documents to your local Social Security office.  If you live in the New York City metropolitan area; Las Vegas, Nev.; Orlando, Fla.; Phoenix, Ariz.; Sacramento, Calif.; or Minneapolis, Minn.; you must apply to your local Social Security Card Center. Be sure to take or mail the original or certified copies of your supporting documents along with the form. We will return your original documents and certified copies right away.
